Poikiloderma of Civatte

is a chronic, benign skin condition characterized by a triad of mottled pigmentation, skin atrophy, and telangiectasia, typically affecting sun-exposed areas such as the sides of the neck, lower face, and upper chest, while sparing shaded areas like under the chin. It primarily affects fair-skinned, middle-aged to elderly individuals, particularly postmenopausal women, and is strongly associated with chronic sun exposure. Other contributing factors include photosensitizing agents (especially perfumes) and hormonal changes, particularly decreased estrogen levels(Postmenopause).

The condition is usually asymptomatic, though some individuals may report mild burning, itching, or flushing. Diagnosis is made clinically based on the characteristic appearance and distribution, with biopsy rarely needed. Key differential diagnoses include cutaneous lupus erythematosus, dermatomyositis, and Riehl melanosis. Although the condition does not cause systemic complications, its cosmetic impact can lead to psychological distress.

Management focuses on photoprotection, including daily use of broad-spectrum sunscreen (SPF 50+) and avoidance of perfumes or fragranced products on affected areas.
