A Non-Ablative Solution for Pigmentation, Texture, and Early Laxity

Faramarz Rafie M.D. , Vancoderm Academy & Vancoderm Clinic (VDCmed)

Thulium 1940 nm laser is a non-ablative fractional laser that targets water in the epidermis, delivering controlled thermal energy to create microthermal zones. It stimulates collagen remodeling, enhances epidermal turnover, and promotes skin renewal without damaging the outer skin barrier.

This laser is particularly effective for epidermal pigmentation treatment, making it ideal for addressing sun damage, melasma, and superficial hyperpigmentation, while also improving skin tone, texture, and hydration with minimal downtime.

Indications:

  • Photoaging (sun damage)
  • Uneven skin tone and texture
  • Superficial pigmentation (melasma, lentigines)
  • Mild to moderate fine lines and wrinkles
  • Enlarged pores
  • Actinic keratoses (early precancerous lesions – FDA-approved)
  •  Dull, dehydrated skin
  •  Elastosis,
  •  Skin laxity

Contraindications:

  • Active skin infection (herpes simplex, bacterial, or fungal)
  • Open wounds or compromised skin barrier
  • Pregnancy or breastfeeding (precautionary)
  • History of keloids or hypertrophic scarring
  • Recent sunburn or tanning (within 2–4 weeks)
  • Use of photosensitizing medications (e.g., isotretinoin within past 6–12 months)
  • Uncontrolled systemic diseases (e.g., diabetes, autoimmune disorders)
  • Skin cancer in the treatment area

Expected Results:

  • Brighter, more even skin tone
  • Reduction in superficial hyperpigmentation
  • Improved skin texture and hydration
  • Tightening of pores and smoothing of fine lines
  • Healthier and more youthful appearance
  • Gradual improvement over several weeks post-treatment

Aftercare Plan:

  • Avoid sun exposure; use broad-spectrum SPF 50+ daily
  • Apply gentle moisturizer and non-irritating cleanser for 5–7 days
  • Avoid exfoliants, retinoids, and acids for at least 5–7 days post-procedure
  • No makeup for 24–48 hours post-treatment
  • Do not pick or scratch peeling/flaking skin
  • Use cold compresses or thermal spring water spray if needed for comfort
  • Hydrate well and avoid heat-inducing activities (hot showers, saunas, workouts) for 48 hours.
  • Based on individual risk factors, we may recommend prophylactic treatment to prevent post-inflammatory hyperpigmentation (P.I.H.).

Sessions & Intervals:

  • Typical course: 3–5 sessions
  • Interval between sessions: Every 4–6 weeks
  • Maintenance treatments: Every 6–12 months, depending on skin aging and exposure

  Downtime:

Downtime with Thulium 1940 nm non-ablative resurfacing is minimal to moderate, depending on the settings used and skin sensitivity.

  • Typical downtime: 1 to 5 days
  • Day 1–2: Mild to moderate redness (erythema), a feeling of warmth or tightness, similar to a light sunburn
  • Day 2–4: Dryness, mild swelling, and bronzing or sandpaper-like texture as the skin begins to exfoliate
  • Day 3–5: Flaking or peeling may occur, especially in pigmented areas (this is part of the rejuvenation and pigment release process)
  • Day 5+: Skin appears brighter, more even-toned, and smoother; most signs of treatment have subsided

Additional Notes:

  • Makeup can usually be applied after 24–48 hours, depending on skin condition.
  • Post-inflammatory erythema may persist slightly longer in sensitive or fair skin.
  • Unlike ablative lasers (CO₂ or Er: YAG), there is no oozing, crusting, or open wound healing phase.

Additional Notes:

  • Results are cumulative and continue improving for 2–3 months post-treatment.
  • Can be combined with other modalities like Microneedling-Dermal Pen, PRP, or Frax: 1550 nm for enhanced outcomes.
